OL-1405 — Hooks installed but not running

What happened

The agent's hooks are installed and correct on disk, and the agent will nonetheless never run them — so nothing is enforced. Three ways to reach it, one code and three remedies: our hook binary does not resolve; the agent has not been told to trust the hook (Codex marks a new or changed hook command for review and skips it until the developer runs /hooks); or an administrative setting suppresses non-managed hooks wholesale.
