OL-1210 — Policy bundle fetch failed

What happened

A poll for the policy bundle failed (network error, 5xx, or 401/403). The previously activated bundle is retained and keeps enforcing — fail-static, never fail-open. Warn level for transient failures, error level for auth failures.

Run openlatch doctor to find the cause, then openlatch system aip refresh to fetch the bundle again.
